PRIZES:
1st Place Winner: $3,000 cash prize

RULES:
1) Video must explain why having your medical records is important to you. Be creative, have fun!
2) Submission should be around 2 minutes in length with a maximum length of 5 minutes.
3) Submit video and required information at https://www.mylinks.com/contest/ .
4) Video entries must be submitted from July 1st through August 31st at midnight (CST).
5) All videos become the property of MyLinks and PatientLink to be used for promotional purposes.
