Contest entry form

To enter, submit an essay or short video explaining why you are a Top Performer in the areas of community involvement, business, entertainment, or sports. Be sure to give examples of how you have bettered your immediate community or the world.

Entries will be judged on the basis of Appropriateness of submission to theme (35%), Level of impact nominee has made on community/world as evidenced in submission (45%), Compelling nature of submission (10%), and Originality of submission (10%).

Steve Harvey will choose one of the top 5 entries each week to receive a Bonus Prize.
